Accessible Information Standard

PREFERRED METHOD OF CONTACT

All organisations that provide NHS services, or publicly funded adult social care must have fully implemented and conformed to the Accessible Information Standard (1605) by 31st July 2016. This means we would like to record your preferred method of contact so that you can receive your health information in a format you can understand.

For most of us our preferred method of contact is our home number or mobile number but for example, if you suffer from hard of hearing or blindness that may not be suitable for you. If you or someone you are caring for wishes us to contact you in another way, please do let us know by informing reception at the surgery.   We will then record your needs by highlighting it on your medical records.

Access to your GP Surgery is changing

The local CCG (Clinical Commissioning Group) are consulting on a proposed new model for Primary Care developed by our GPs to improve access to GP services in East Lancashire. After initial feedback from our patients, our vision is for same day access, 8am-8pm 7 days a week to a GP wherever you are in East Lancashire. This consultation is about the principles of delivering extended GP access for you. The CCG (Clinical Commissioning Group) would like to hear what you think.  Please click here for more information and to complete a short survey. We strongly encourage our patients to complete this survey and help shape the future of primary care.

Friends and Family Test

As you may be aware, as part of the GP Contract for 2014/2015 it is a requirement from the 1st December 2014 for all practices to ask patients who attend the surgery a mandatory 'Friends and Family Test' question. This is a feedback tool which enables people who use NHS services to provide feedback on their experience. If you have recently visited the surgery and would like to let us know about your experiences please click on - 'Take The Patient Survey' below. Thank you.

Would you like a say into the services that we offer at the surgery? Is there something we are doing which you think we could do better? Why not become a member of our Patient Participation Group. VISITING THE PRACTICE
Please only attend the surgery if instructed to do so by a member of the practice team.

TELEPHONE CALLS
As you may be aware the surgery is receiving an extremely high number of telephone calls. We can assure you that the phone lines are working completely fine but you may have to wait in the queue for longer than usual. We will answer your call as soon as we can.

PRESCRIPTION REQUESTS
Please do not come into the surgery with prescription requests/queries. If you have an issue please telephone the surgery after 10.00am. We will also temporarily be taking prescription requests over the telephone or by email at pendleviewmedicalcentre@nhs.net.

You will be asked to nominate a pharmacy so that we can send the prescription straight to them without anyone having to collect them from the surgery. Any one who does not nominate a pharmacy will be asked to provide a stamped self addressed envelope so we can post your prescription to you

APPOINTMENTS
We are currently in the process of changing all of our pre booked appointments to telephone appointments. You should either receive a text message, phone call or letter. If you have an appointment booked and you have not had any contact please ring the surgery for more information.

SICK NOTES
We are currently posting out/sending digitally all sick/fit notes to patients. Please do not come to the surgery to collect or request your sicknote. If you need a sicknote, please telephone the surgery. If you are self isolation and cannot work from home you can get an isolation note from NHS111 https://111.nhs.uk/isolation-note

AND FINALLY...….
Please remember that we are only human and we have feelings. Please remember to treat our staff with respect. Over half of our patients do not have a nominated pharmacy set up and so cannot receive electronic prescriptions, if you are one of these please consider nominating a pharmacy as soon as possible. You can nominate any pharmacy to receive your electronic prescriptions, near home or work or where you do your shopping, most pharmacies offer free delivery so if you do have to self isolate there would be no disruption to you getting your medication. Many pharmacies will accept nominations over the telephone.

In this difficult time please allow up to 72 hours for your repeat prescriptions to be processed by the surgery.

To follow on from yesterday’s guidance issued by the Government, NHS England and Public Health England, Pendle View Medical Centre has made the decision to put in place some extra emergency measures, to help minimise the risk to both our patients, staff and their families.  At the moment, all routine work will be suspended. Any prebooked face to face appointments, for ALL clinicians, will be changed to either telephone or video appointments, where appropriate. We will be contacting patients with prebooked appointments regarding this. Prescription requests will still be processed, however where possible, we would like to be able to send prescriptions electronically direct to the pharmacy. We use this process already for lots of our patients. Where possible, please inform us of your regular pharmacy.

We will keep you up to date of any further developments, as and when they occur. Our main priority is to keep our patients as safe as possible.

In this difficult time please allow up to 72 hours for your repeat prescriptions to be processed by the surgery.

We do not have any routine appointments available for any

GP or Nurses

If you have a persistent cough or a temperature please self isolate for 7 days, do not come to the surgery, more advice is available from NHS 111 on line.
